Patient enquiries your Bangalore clinic never answers: the follow-up gap that quietly wastes your ad spend
Most clinics in Bangalore do not have a problem attracting enquiries. They have a problem answering them. Between a quarter and two fifths of calls to a busy practice go unanswered during peak hours, and a patient who cannot reach you rings the next clinic on the results page within a few minutes. Fixing who answers, how fast, and on which channel usually returns more booked appointments than raising your ad budget.
Where patient enquiries arrive
Not all of them are phone calls, and that is the first thing most front desks get wrong. Across the small Bangalore clinics we work with, a typical week splits roughly 55 percent phone, 30 percent WhatsApp, 10 percent website form, and the rest through Instagram messages and the Google Business Profile call button.
That matters because the non-phone channels usually have no owner. The website form goes to an email nobody has opened since the site launched. WhatsApp sits on the receptionist's personal phone and goes home with her at 8 pm. A clinic can look well staffed and still leave 4 in 10 enquiries with nobody responsible for them.
How fast should a clinic call back?
Inside five minutes for a first enquiry, and the same working day at the very worst. In the enquiry logs we reviewed for North Bangalore clinics between February and June 2026, first contact within five minutes converted to a booked appointment roughly four times as often as first contact after 30 minutes.
The reason is not urgency in a medical sense. A patient comparing spine surgeons or dermatologists rarely contacts only one clinic. They send three enquiries in the same sitting, and the first clinic to reply gets to frame the conversation. By the time your receptionist calls back at 4 pm about a 10 am WhatsApp message, the patient has been given a slot somewhere else.
Why enquiries get missed even with someone at the desk
Look at when they arrive. A three-doctor polyclinic in Yelahanka logged 148 missed calls over one week in April 2026. Of those, 61 landed between 1 pm and 2:30 pm while the desk was at lunch, and another 34 came after closing, when patients finally sit down to plan the week.
A single receptionist handling a counter queue, a phone line and a WhatsApp inbox will drop something in those two windows every day. That is a staffing and routing question rather than a marketing one, but it shows up in your marketing report as a poor cost per acquisition.
What a missed enquiry costs when you are running ads
Put a number on it. At an average cost per click of about Rs 55 for "skin specialist Yelahanka" through Q2 2026, and an enquiry rate of 7 percent on a decent landing page, one enquiry costs you close to Rs 790. Miss 3 in 10 of them and the real cost of an enquiry that got answered is Rs 1,130, before anyone has walked in.
Most clinic dashboards never show this, because they count clicks, form fills and calls initiated. They do not count calls connected, so the gap hides between the two. Compare the calls your ad platform reports against the number your phone system logged as answered, which is the tracking side we covered in our guide to Google Ads conversion tracking in Bangalore.
What a working follow-up system looks like for a small clinic
It does not need software. It needs one number, one owner, and one rule.
One number means every channel points at the same line. The Google Business Profile, the website, the ads and the signage all show a single clinic number that is on WhatsApp Business, so a missed call and a message land in the same place. One owner means a named person holds the enquiry inbox for each shift, including the lunch window. One rule means a callback promise you can keep, written on the website and in the WhatsApp away message: enquiries before 7 pm get a reply the same day, later ones by 11 am the next morning.
WhatsApp Business covers the rest at no cost. Labels for new enquiry, follow-up pending and appointment booked give you a working pipeline. A missed call automation that sends your timings and a booking link recovers a good share of the after-hours calls that used to vanish. One Yelahanka clinic added that in March 2026 and booked 38 appointments over the next eight weeks from calls it would previously have lost.
Then keep a log: date, channel, treatment asked about, time to first reply, outcome. Six weeks of that tells you whether your problem is enquiry volume or enquiry handling, and only one of those is solved by spending more on ads.
Being found is half of it, being reachable is the other half
Search visibility and enquiry handling are the same project. A patient searching "spine surgeon North Bangalore" at 10 pm may land on a practice page like Neospinex, read Dr. Akshay Hari's credentials, and message immediately. If that message goes to a phone in a drawer, the visibility work is wasted. A multispecialty clinic like Chitra's Lifeline Clinic in Yelahanka New Town publishes its hours plainly, Monday to Saturday, 11 am to 8 pm, so a patient writing at 9 pm knows when to expect an answer.
Clinics that get this right treat the enquiry log as part of their marketing data. If you want help joining the two halves, Studio Happens builds the search and ad side around what the front desk can handle, and keeps it affordable enough for a two-doctor practice to run continuously rather than in bursts.
Frequently asked questions
How quickly should a clinic reply to a WhatsApp enquiry?
Within five minutes during clinic hours, and by the next morning for messages sent after closing. Patients usually message two or three clinics in the same sitting, so the first clear reply tends to get the appointment. An automated away message with your timings holds the enquiry until a person can answer it.
Is it allowed for a clinic in India to message patients on WhatsApp?
Yes, for appointment coordination, reminders and answering questions a patient asked first. What the medical council code restricts is solicitation, testimonials and claims of superiority. The safe line is factual and responsive rather than promotional. We wrote about what a Bangalore clinic can publish online in more detail.
Do we need clinic software to fix this, or will a phone and a notebook do?
A notebook works for a single-doctor practice with under 20 enquiries a week. Above that, WhatsApp Business labels plus a shared spreadsheet cover most needs at no cost. Paid clinic software is worth it when you have multiple doctors and want automated reminders to cut no-shows.
Which time of day do most clinic enquiries get missed?
The lunch window between 1 pm and 2:30 pm, and the hour after closing. Both are periods when patients are free to call and front desks are not. Covering those two windows with a rota or an away message recovers most of what is currently lost.
